Casey King enters the most dramatic stage of his weight-loss journey in the latest episode of TLC's Family by the Ton, the spinoff of its hit reality series My 600-lb Life. King, now down to 631 pounds after starting the season around 700 pounds, has an emotional goodbye with his family before he has to check in for weight loss surgery.

"I'm losing it. I want to run out of the room, hop on a train, and never come back," King, 34, says. "But I can't back out now."

King started the season in a fairly desperate situation. He generally spent his days living with his father on his bed playing games and eating, rarely moving outside. "I was just eating 'til I'm dead," King said then. "I never would've thought at 34 I'd be living with my father, I'd have no job, have no real money, and just be playing video games all day and eating."

But in addition to being a charismatic and candid reality star, King has been steadfast in his commitment to changing his overall lifestyle and improving his health. While he has no plans to fully give up the video games, he followed a strict new diet and saw a personal trainer for the first time in his life. But those didn't quite prepare him for surgery.

Talking to his parents once he's on the hospital bed, King breaks into tears. "I'm just like terrified. I hate that I'm so scared," he says. "I can't help it."
