In a new video posted to her Instagram feed, dermatologist Sandra Lee, MD, expertly wields her extraction tool on a man with a super congested nose-and I'm not talking about the sniffles here. The patient's schnoz is riddled with a number of pesky blackheads, which Dr. Pimple Popper dispatches to surprisingly goopy effect. (Seriously, how do such small blemishes contain so much goop!?!)

"I'm not actually pressing hard," Dr. Pimple Popper says to the man's female companion, who's in the room observing. "They're ready to come out."
"Is it because he's so oily?" she asks.
"Yeah, he's got a little thickening of his skin here," Dr. Pimple Popper responds. "So it's all really going to come out very easily."
Easily-and with some pretty impressive sound effects too!
In case you didn't know, blackheads form when hair follicles or pores are clogged-often by dead skin or oil.
